Teachers and Staff use this form to officially request monetary funds from the Freedom Elementary PTA that will be used to support a program or activity that is directly in alignment to the PTA’s Missions and Goals. |

The purpose of this form is to officially request monetary reimbursement from the Freedom Elementary PTA for pre-approved expenses incurred on behalf of the PTA. |
